- Shelter such as rent or mortgage,
- Prescription drugs, and,
- Transportation.
Appointsment are necessary to apply for financial assistance. Appointments may be made Monday through Friday from 9:00 AM to 12:30 PM.
Applicants should bring ...
- Proof of address in the Shively Area Ministries service area,
- A photo ID, and,
- The Social Security cards of household members.
- Depending upon assistance needed it may also be necessary to bring a brown bill, an eviction notice or a prescription.
